COVID-19 Vaccines Available
Our team is excited to be partnering with Southeastern Utah Health Department to offer the COVID-19 vaccine to community members ages 5+ beginning November 9, 2021. We are offering the Pfizer vaccine, which has received FDA approval. We also offer the Pfizer vaccine for ages 5-11 that has received FDA Emergency Use Authorization. Vaccines will be provided by appointment only.
To schedule your vaccine appointment, call us at 435.381.2305. We will schedule for the number of vaccines available.
Our COVID-19 Vaccine Clinic will be held during normal business hours. Please note: days and hours are subject to change based on vaccine availability.

Once you have an appointment scheduled, please complete the Vaccination Administration Record paperwork. Bring this paperwork to your appointment.
